Please note that we are not yet set up to accept online payments and this web site does not support secure transactions. Please do not email confidential information.

2.  Print and mail : download, fill out and print the registration form found here.

A .pdf (Adobe) file will open or be available to save.  Please print, complete and mail the form along with your cheque.  Please do not send cash through the mail.  Please note that you may need to obtain or update your free Adobe Reader - available here.

3.  In person : Stop by CMHA Kingston at 6 Ordnance Street or call (613) 549-7027 during business hours. 

We will be more than happy to accept cash or a cheque from you at the branch.
